Accelerate to 100m s going straight up. turn 5 10 degrees east. turn more for higher twr, less for lower twr. do the turn smoothly, but reasonably quickly, as the faster you go, the more loses it will incur. wait until your velocity lines up with rocket's orientation and switch sas to prograde.
